PUP Introduces Nurse Care Coordinator Program

This program was developed to assist our Valued Providers with transition of care with our Members, your patients. Our Nurse Care Coordinators will be collaborating with your case managers, care coordinators, physicians and nursing staff to assist with transition of care plans for our Members. If you have a PUP Member that you think may benefit from our Care Coordination Program; call our Nurse Care Coordinator/Nurse Hotline at 1-866-773-1071 Monday through Friday from 8:30 am to 5 pm.

Health Literacy Techniques for Providers

In accordance with URAC standards, PUP has developed specific training regarding health literacy and plain language in the health care setting. This material provides insight and techniques that can help improve patients’ health literacy so they better understand, and act on, health information.


Cultural Competency; Culturally & Linguistically Appropriate Standards (CLAS)

This material stresses the importance of eliminating language and cultural barriers in health care. The CLAS Standards were established by the U.S. Health and Human Services Department, Office of Minority Health, to define legal and regulatory requirements for providing health services to individuals with limited English proficiency.
